Note:  This is a quick reference chart. In some cases these forms may overlap – such as a four day camp outside of the country – both the Category 3 and International Travel columns apply.

*PLEASE NOTE: It is imperative that you submit this form immediately after the incident happens; please submit one copy to National, according to the instructions on the form, and a second copy to the Pacific Coast Council Office, Attention Iqbal Lalany at (fax) 604-879-5725. Failure to do so may result in a loss of insurance coverages.
