World Scout Jamboree 2011 - Forms
Registration for the 2011 World Jamboree is now closed. However, if you’d like to review the forms or the application process, they are available below. For those of you who have yet to submit your Health and Fitness Statement, please be sure to do so ASAP as it is a required document to attend the Jamboree.
- Read the Application Instructions in full before filling out any of the forms.
- Make sure you fill out the correct forms as per the Application Instructions, and check off the box at the top of the Registration form that coordinates with your position (i.e. IST, Youth or Leader).
- Make sure you meet all of the participation criteria as outlined on the Registration forms.
- If you are registering a Patrol (1 adult and 9 youth), have each member of that patrol fill out one form each, and send them all in together along with all of the registration payments.
- Send in all forms and deposit payments together to:

Deadline for registration was February 28th, 2011. Any applications/payments received (post-marked) after this date will be charge an additional $65.00 fee.

Please note that all forms are in interactive PDF format; you can therefore either fill out the forms online by typing directly into the spaces provided then print them, or you can print the forms first, then fill them in by hand.
